HomeCarePlus
The North Penn Visiting Nurse Association emphasizes excellence in home and community care and provides a diverse array of services to residents of the North Penn Community and surrounding areas. This has been our tradition since 1919.
What we believe:
- Quality care given along the life to death continuum is essential in delivering comprehensive health services to the community.
- An interdisciplinary approach to healthcare enables us to provide individual, efficient and cost-effective alternatives to institutional care.
- Health promotion, health education, health maintenance and health restoration are the framework for our care.
Our services:
Hospice and Skilled Services
There's no place like home in times of health and prosperity, and perhaps even more so in times of illness and medical recovery. The North Penn Visiting Nurse Association provides high quality home and hospice care. Professional staff members work closely with physicians to develop care plans to treat patients who have recently been discharged from the hospital, are bedridden or temporarily disabled, are suffering from multiple medical problems or are terminally ill. Care is provided 24 hours a day, seven days a week. Volunteers can provide companionship to patients and respite to caregivers. For patients who are unable to remain at home, the hospice program provides care in both the hospital and nursing home settings. Other services include: physical, occupational and speech therapy, homemaker/home health aides, social work and bereavement counseling. Most third-party payment plans, including Medicare and HMO, are accepted. For those without insurance coverage, a sliding scale payment system is available. Many families designate our hospice as a memorial contribution in lieu of flowers. For more information, please call 215-855-8297.
Adult Day Services
Adult Day Centers are conveniently located in Lansdale on the Central Montgomery Medical Center campus and in Souderton on the grounds of the Indian Valley Senior Adult Activity Center. Qualified, well-trained and caring staff members provide participants with a wide range of health, rehabilitation and social services. Meaningful, enjoyable and personalized activities are hallmarks of both the Lansdale and Souderton Centers. Socialization and creativity are encouraged to reduce feelings of isolation and depression. Services can assist persons ages 18 and up who have mobility or physical problems and/or mental impairments such as dementia. The centers are open from 7:00 AM to 5:30 PM, Monday through Friday with extended hours available. Transportation can be arranged through Suburban Transit Network. For participants who qualify, wheelchair-accessible vans and cabs are available at no cost or for a small fee. Third-party subsidies may be available for those who qualify. For enrollment information, please call 215-855-8296 for Lansdale or 215-703-0523 for Souderton.
Meals on Wheels
The NPVNA Meals on Wheels program provides two nutritious meals a day -- a hot lunch and cold evening supper -- to homebound adults ages 18 and up who are unable to shop for food and cannot prepare meals as a result of a disability, temporary illness or weakness following hospitalization. Volunteers prepare food and pack it in insulated chests for delivery to homes in the North Penn area five days a week. The cost of this program is based on ability to pay. Funding is provided by the North Penn United Way, the Montgomery County Aging and Adult Services organization and private donations. Tax-deductible donations of all sizes are gratefully accepted. To apply for program benefits or for volunteer information, please call 215-855-8296.
